Program Directors
Lauren Oshman, MD, MPH
Program Director
Dr. Oshman is a family physician and clinical associate professor in the University of Michigan Department of Family Medicine. Prior to joining the University of Michigan in 2020, she practiced for five years in Concord, New Hampshire, then served as a residency faculty member and vice-chair for quality at NorthShore University Health System in Chicago.
Heidi L. Diez, PharmD, BCACP
Co-Director
Dr. Diez is a clinical pharmacist specialist and clinical associate professor in the University of Michigan College of Pharmacy. She graduated from the University of Michigan (UM) College of Pharmacy (2004) and completed a community practice residency at the University of Washington and Kelley Ross Pharmacy in Seattle, WA (2005).

Kara Mizokami-Stout, MD, MS
Endocrinology Program Director
Dr. Kara Mizokami-Stout, MD, MSc is an Assistant Professor of Internal Medicine in the Division of Metabolism, Endocrinology and Diabetes at the University of Michigan and the Lieutenant Colonel Charles S. Kettles VA Medical Center in Ann Arbor, MI. Dr. Mizokami-Stout’s clinical interests include type 1 and type 2 diabetes. Her research interests include diabetes health services research with a particular focus on 1) prevention and management of diabetes complications and 2) implementing technologies to improve diabetes self-management and quality-of-life.
Lynn Ang, MBBS
Endocrinology Content Expert
Dr. Lynn Ang is a board-certified endocrinologist and Clinical Associate Professor in the Division of Metabolism, Endocrinology, and Diabetes within the Department of Internal Medicine at the University of Michigan. She serves as a co-director of the Hospital Intensive Insulin Program and chairs the Inpatient Glycemic Management Subcommittee. Her clinical and research interests include optimizing the management of hyperglycemia in both inpatient and outpatient settings, developing protocols for hospital glycemic guidelines, identifying biomarkers to predict the development or progression of diabetic complications, and detecting the earliest stages of these complications to prevent disease progression.
Jennifer Iyengar, MD
Endocrinology Content Expert
Dr. Jennifer Iyengar, MD is a board-certified endocrinologist and Clinical Associate Professor at the University of Michigan in the division of Metabolism, Endocrinology, & Diabetes. She is the Primary Quality Improvement Person (PQuIP) for the Michigan Medicine (MM) endocrinology division and serves as a member of the MM Internal Medicine Quality Council and the MM Outpatient Diabetes Quality Improvement (QI) Committee.
Michael Heung, MD, MS
Nephrology Program Director
Dr. Mike Heung, MD, MS is a nephrologist at the University of Michigan, where he serves as the Associate Chief of Nephrology and chairs the nephrology quality improvement committee. His primary clinical and research interests are acute kidney injury (AKI) and critical care nephrology. As an investigator at the University of Michigan Kidney Epidemiology and Cost Center (UM-KECC), he examines AKI measures and outcomes on multiple federally funded projects.
Rajiv Saran, MD, MBBS, MS
Nephrology Content Expert
Dr. Rajiv Saran, MBBS, MS, is a nephrologist at the University of Michigan, where he serves as the Florence E Bingham Research Professor of Nephrology, a Professor of Internal Medicine, U-M Medical School, and a Professor of Epidemiology, at the U-M School of Public Health. Dr. Saran is an internationally recognized expert in kidney disease research – specifically, in the area of kidney disease surveillance and epidemiology.
Julie Wright Nunes, MD, MPH

Dr. Wright Nunes, MD, MPH, is a nephrologist at the University of Michigan, where she serves as an Associate Professor of nephrology. Dr. Wright Nunes' research focuses on optimizing patient kidney disease education and self-management across the continuum of care.

Jonathan Gabison, MD
Dr. Jonathan Gabison, an Assistant Professor in Family Medicine at the University of Michigan, stands at the forefront of chronic disease management with a focus on obesity and diabetes. Board-certified in Obesity Medicine, his expertise extends to the development of innovative programs and curricula aimed at improving the treatment and understanding of obesity and related metabolic diseases within primary care settings.
Dina Hafez Griauzde, MD, MS
Dr. Dina Hafez Griauzde, MD, MS’s research interests include understanding both the provider and patient factors that influence diagnosis and treatment of prediabetes. Additionally, she is interested in the implementation and evaluation of novel interventions focused on primary obesity prevention in infancy and early childhood.
Joyce Lee, MD, MPH
Dr. Joyce Lee, MD, MPH’s research focuses on the use of emerging technologies (mobile technology, data visualization, participatory game design, social media) and the creation of learning health systems (incorporating health systems design and design/ethnography) for improving outcomes in type 1 diabetes.
Coordinating Center Team
Jackie Rau, MHSA, PMP
Program Manager
Jackie Rau, MHSA leads the overall management, performance, and coordination of the Michigan Collaborative on Type 2 Diabetes (MCT2D) Program and Coordinating Center Team. Jackie began her career on a multidisciplinary healthcare team at Michigan Medicine and has since worked on quality improvement projects in insurance and provider organizations.
Jake Reiss, MHSA
Associate Program Manager
Jake’s role involves working closely with participating sites to help disseminate and implement quality initiatives, developing materials and resources for healthcare providers and their patients, and organizing trainings for clinicians and their teams. He has a Master of Health Services Administration from the University of Michigan.
Neha Bhomia, MHI
Data Lead
Neha manages the interoperability between the different systems, integration of device data, and generation of datasets from the CQI Data Hub. Leveraging her background as a healthcare provider, Neha converges healthcare and technology to improve quality of patient care. In addition to her work with MCT2D, Neha serves as Applications Systems Programmer Analyst at the University of Michigan and as Adjunct Lecturer in Information at the University of Michigan School of Information.
Ryan Carpus
Software Developer Senior
Ryan is the web applications developer for the Michigan Collaborative for Type 2 Diabetes (MCT2D). Ryan has a Bachelor’s and Master’s degree in Civil Engineering, both from the University of Michigan.
Rina Hisamatsu, MPH, RDN
Registered Dietitian
Rina Hisamatsu, MPH, RDN, is a registered dietitian who joined Michigan Medicine in the summer of 2021. Her role is split between lead dietitian for The Michigan Collaborative for Type 2 Diabetes and primary care dietitian at Domino’s Farms Family Medicine.
Noa Kim, MSI
Informatics Design Lead
Noa is the Clinical Informatics Design Lead for the Michigan Collaborative for Type 2 Diabetes (MCT2D) and Healthy Behavior Optimization for Michigan (HBOM). They strive to design stories, services, and systems that make room for the humanity of healthcare experiences.
Amy Kuehls
Administrative and Project Coordinator
Amy joined the MCT2D team in March 2024. As AC/PC, Amy supports event planning for MCT2D regional and collaborative-wide in-person meetings and provides leadership and support on cross-functional projects with the design, informatics, data, and operation teams.
Kaitlyn Watson, BS
Statistician
Kaitlyn Watson is a Statistician for MCT2D, specializing in statistical programming and data management. Her main responsibilities include creating reports, performing statistical analyses, and collaborating with various teams to fulfill data-related requests. Before joining the team, Kaitlyn earned her B.S. in Statistics from Michigan State University and worked as a Data Manager for a child health study at the College of Human Medicine. She will begin pursuing her Master's in Applied Data Science at the University of Michigan in Winter 2025.
Julian Weisensel, BFA
Multimedia Design Specialist
Julian is a Multimedia Design Specialist for the Michigan Collaborative for Type 2 Diabetes. His role involves working with the design and informatics team. He studied information technology and computer science before committing to graphic design. Prior to his role in MCT2D Julian worked at the University of Michigan, University Health Service, Wolverine Wellness as a graphic designer in the communications team.
Lypha Yang, BS
Administrative Specialist
Lypha is originally from Arizona and is an alumni of Arizona State University, where she obtained her B.S. in Communications. Before joining the Michigan Collaborative for Type 2 Diabetes, Lypha was an Executive Assistant at a local tech start up, where she supported several Executives.
Larrea Young, MDes
Multimedia Design Project Manager
Larrea is a designer, illustrator, and educator creating human-centered design solutions with a focus on food, health, and the environment. She is passionate about using design research to create solutions that help people live happier, healthier lives.

Caroline R. Richardson, MD
During her time at the University of Michigan, Dr. Richardson served associate chair for research programs in the Department of Family Medicine and as the director of the Veterans Administration Diabetes Quality Improvement Initiative (QUERI). Additionally, she was the founding director of MCT2D. Dr. Richardson currently serves as Chair of Family Medicine at the Warren Alpert Medical School of Brown University. In addition, she serves as editor-in-chief of the Annals of Family Medicine.
Rodica Pop-Busui, MD, PhD
Dr. Rodica Pop-Busui, MD, PhD is the Larry D. Soderquist Professor of Diabetes, vice chair for Clinical Research in the Department of Internal Medicine, and the director of Clinical Research,
Mentoring and Development of the M-Diabetes. Her research interests involve chronic
complications of diabetes, diabetic peripheral and cardiovascular autonomic neuropathy,
and implementing the use of technologies in treating type 1 diabetes and type 2 diabetes.
