April PO calls are going to be canceled for MCT2D this month as a result of the regional meetings taking place. We will update the invites shortly.
Five of the seven regional meetings take place in April. Please make sure that your clinical champions are registered three weeks in advance of the meetings. The final two regional meetings will take place in May.

April 23rd: Grey Wolf/Kalamazoo
April 24th: Blue Gill/Grand Rapids
Collaborative wide meeting registration will open this week. An administrative lead and the PO clinical champion are required to attend per the PO scorecard requirements.
April 26th: (12pm): There will be a learning community event on “Integrating Low Carbohydrate Diet with Continuous Glucose Monitors” led by Dr. Dina Griauzde.
VBR
Please remind your providers about upcoming VBR deadlines (June 1st). There are less than two months remaining in the current VBR year and many physicians and practices who still need to complete their learning community requirement.
The final component of the PO level quality improvement initiative VBR –“Lessons Learned/Reflection” will be available later this month for you to view in the admin portal. This is also due on June 1st but we wanted to give you some time to review at any meetings that you may have with practices prior to that due date.
POs with participating specialists: PCP and specialist partnerships are due in the administrative portal today, April 1st.
Reminders/FYIs
Quarterly data reports will be distributed later this month, including updated A1C and BMI submission rates by PO. We are still evaluating if it makes sense to include these rates as part of the final PO scorecard, but wanted to provide them on an informational basis.
PCP recruitment remains open through April 19th, 2024. MCT2D has the capacity to bring on an additional 75 primary care practices. Please submit any primary care site interested in joining on the administrative portal by this date.

Reminder: Your PO scorecard ensuring registration by your clinical champions three weeks prior to their meeting date is associated with 10 points. The first regional meeting takes place in Livonia on April 9th.
RECRUITMENT
You will be able to nominate eligible PCP practices in the administrative portal later this month.
Reminder: MCT2D is able to bring on 75 additional primary care practices. If we receive more nominees than that we will use the criteria shared in the January PO monthly call to determine how to prioritize onboarding.
Recruitment will be open through April 19th, 2024.
MCT2D is planning on hosting a practice level introduction to MCT2D. We would encourage you to share the link to register with any sites that may be considering joining. We will also discuss other recruitment resources at the upcoming February PO calls.
REMINDERS & FYI
In January, we distributed a list with contact information for other PO leadership working on the same QI focus initaitive. Please feel free to reach out to others to discuss their process changes or if you think you can share something helpful with them.

The next MCT2D dashboard refresh will take place on 2/6/2024 and will have paid clinical and claims data through 11/30/2023.

VBR
There are five months left in this VBR cycle.
On December 28th, reminder emails were sent letting PCP clinical champions and PO level contacts know about physicians who still need to meet the learning community requirements.

Reminder: Physicians are now able to access the MCT2D Admin Portal for their practice. We have had over 50 sign-ups so far. Please remind practices that they can gain access to the portal
Specialist Updates
In mid-January, we will be confirming specialist VBR recipients for the next VBR cycle, set to begin on 3/1/2024. You will be able to view a list of the physicians that we will be submitting for VBR, and we will ask you to review and approve or submit edits to that list prior to our submission to BCBSM, in order to ensure accuracy.
