May 2024 Monthly PO Call and Digest
Meetings
  • During our May PO calls (Wednesday May 8th and Monday May 13th) we will be asking for PO feedback on the following topics: the pre-populated reports that we will include as part of our dashboard redesign and our proposed performance measure that will begin on September 1, 2024. Please consider what type of standard reports would be helpful for you to view on the dashboard.
  • The final two spring regional meetings will take place in May: 
    • May 8th: Saginaw/Riverwalk Region
      • May 9th: Gaylord/Sleeping Bear Dunes Region
  • Collaborative wide meeting registration is closing soon, please plan to register by Friday, May 3rd so that we have an accurate count for the venue.
  • Learning Community Event: Friday, May 25th, 12pm-1pm- Member Panel on “Implementing CGM Interventions” where participants from physician organizations who chose to focus on continuous glucose monitors for their quality initiative focus will be sharing their experiences.
VBR
  • Please remind your providers about upcoming VBR deadline of June 1st to complete all primary care VBR requirements. 
  • Next week we will be distributing an additional opportunity to meet the practice-level learning community VBR by submitting a brief survey about support needed related to DSMES, pre-diabetes, uncontrolled diabetes, and other topics. 
  • The final component of the PO level quality improvement initiative VBR –“Lessons Learned/Reflection” is available for you to view in the admin portal. This is also due on June 1st but we wanted to give you some time to review at any meetings that you may have with practices prior to that due date. 
  • POs with participating specialists: Details on the specific goal and approach for your PCP-SCP partnership are due in the administrative portal on June 14th. Prior to submitting these goals, the expectation is that you will have met with your specialist practice and your PCP practice, ideally together, to develop priorities and an approach to this partnership.
Reminders/FYIs
  • PCP recruitment for MCT2D closed April 19th. 59 new sites were enrolled in the program for a September 1st start date. 
  • The Spring PGIP list has been imported into the MCT2D admin portal and changes in practice and physician alignment should be reflected there.
  • Version 1.0 of the virtual MCT2D Coverage Checker will be available at the end of May.

April 2024 Monthly Digest
Meetings
  • April PO calls are going to be canceled for MCT2D this month as a result of the regional meetings taking place. We will update the invites shortly.  Five of the seven regional meetings take place in April. Please make sure that your clinical champions are registered three weeks in advance of the meetings. The final two regional meetings will take place in May.
    • April 9th- Blue Jay/Livonia
    • April 11th- Badger/Ann Arbor
    • April 16th: Black Bear/Troy
    • April 23rd: Grey Wolf/Kalamazoo
    • April 24th: Blue Gill/Grand Rapids 
  • Collaborative wide meeting registration will open this week. An administrative lead and the PO clinical champion are required to attend per the PO scorecard requirements.  April 26th:  (12pm): There will be a learning community event on “Integrating Low Carbohydrate Diet with Continuous Glucose Monitors” led by Dr. Dina Griauzde.
VBR
  • Please remind your providers about upcoming VBR deadlines (June 1st). There are less than two months remaining in the current VBR year and many physicians and practices who still need to complete their learning community requirement. 
  • The final component of the PO level quality improvement initiative VBR –“Lessons Learned/Reflection” will be available later this month for you to view in the admin portal. This is also due on June 1st but we wanted to give you some time to review at any meetings that you may have with practices prior to that due date. 
  • POs with participating specialists: PCP and specialist partnerships are due in the administrative portal today, April 1st.

Reminders/FYIs
  • Quarterly data reports will be distributed later this month, including updated A1C and BMI submission rates by PO. We are still evaluating if it makes sense to include these rates as part of the final PO scorecard, but wanted to provide them on an informational basis. 
  • PCP recruitment remains open through April 19th, 2024. MCT2D has the capacity to bring on an additional 75 primary care practices. Please submit any primary care site interested in joining on the administrative portal by this date.

Spring Regional Meetings

  • Regional meeting registration will open the first week of February.
  • Reminder: Your PO scorecard ensuring registration by your clinical champions three weeks prior to their meeting date is associated with 10 points. The first regional meeting takes place in Livonia on April 9th.


RECRUITMENT

  • You will be able to nominate eligible PCP practices in the administrative portal later this month.
  • Reminder: MCT2D is able to bring on 75 additional primary care practices. If we receive more nominees than that we will use the criteria shared in the January PO monthly call to determine how to prioritize onboarding.
  • Recruitment will be open through April 19th, 2024. 
  • MCT2D is planning on hosting a practice level introduction to MCT2D. We would encourage you to share the link to register with any sites that may be considering joining. We will also discuss other recruitment resources at the upcoming February PO calls.


REMINDERS & FYI

  • In January, we distributed a list with contact information for other PO leadership working on the same QI focus initaitive. Please feel free to reach out to others to discuss their process changes or if you think you can share something helpful with them. 
  • Quarterly data reports were distributed to each PO on January 19th. 
  • The next MCT2D dashboard refresh will take place on 2/6/2024 and will have paid clinical and claims data through 11/30/2023.

  • January 22nd: PO Informational Session for NEW physician organizations who are considering joining MCT2D. As a participating PO, you do not need to attend.
VBR
  • There are five months left in this VBR cycle.
  • On December 28th, reminder emails were sent letting PCP clinical champions and PO level contacts know about physicians who still need to meet the learning community requirements.
  • On December 29th, reminder emails were sent letting PCP clinical champions and PO level contacts know if they have not yet met the practice level learning community requirement.
  • Reminder: Physicians are now able to access the MCT2D Admin Portal for their practice. We have had over 50 sign-ups so far. Please remind practices that they can gain access to the portal
Specialist Updates
  • In mid-January, we will be confirming specialist VBR recipients for the next VBR cycle, set to begin on 3/1/2024. You will be able to view a list of the physicians that we will be submitting for VBR, and we will ask you to review and approve or submit edits to that list prior to our submission to BCBSM, in order to ensure accuracy.
  • Due January 12th:
    • Cohort 1 Endocrinologists (began participation in January 2022): Each individual physician needs to view the endocrinology clinical champion meeting recording and answer questions related to the content. Link is here

December 1: Halfway through the PCP VBR cycle. We suggest checking in and sending reminders to physicians who still have their learning community requirement to complete. MCT2D will share an updated list of events and the full list of ways these requirements can be met for you to share out with your practices in early in 2024.

PO Information:

Meetings
  • The final two fall regional meetings are taking place in Kalamazoo/Grey Wolf (November 7th) and Grand Rapids/Blue Gill (November 8th).
  • The PO monthly calls this month are taking place on Wednesday, November 8th at 2pm and Monday, November 13that 11am.
  • On November 17th, Dr. Dina Griauzde, a primary care physician board-certified in obesity treatment, will be giving a presentation on weight loss medications. Please share this with your practices, as this is a topic we get many questions about and there seems to be significant interest from the sites. CME is offered for attendance at the live session and learning community VBR is available for both the live and recorded sessions.
VBR
  • There are multiple requirements coming up this month related to the Quality Initiative Focus VBR metrics
    • First, we are asking that you convene a meeting with your practice clinical champions to discuss the target rate and the process changes that each practice will implement.
    • Then, by November 15th, submit your target rate in the administrative portal.
    • Finally, by November 30th, submit the proposed process changes in the administrative portal as well.
  • We will be scheduling the PO-level calls for each Quality Initiative focus for sometime in January. Invites will be sent out this month. The intent of these meetings is to share amongst POs who are working on the same focus.
Specialist Updates
  • The nephrology clinical champion call is taking place on Thursday, November 16th at 5:30pm. Attendance at this virtual call from the clinical champion or a substitute nephrologist from the practice is required for the VBR.
  • The endocrinology clinical champion call is taking place on Wednesday, November 29th at 5:30pm. Attendance at this virtual call from the clinical champion or a substitute endocrinologist from the practice is required for the VBR.
  • Please remind year one endocrinology and nephrology sites that care coordination summaries and case summaries (respectively) are due on January 15th. This represents an extension from the previous due date as BCBSM has extended the timeline on when we need to submit specialists who have met the VBR requirements to them.
  • For POs that are planning to enroll endocrinology practices and nephrology practices, all recruitment is due on Friday, November 3rd.
Patient Empowerment Toolkit
  • The Patient Empowerment Toolkit announced at the regional meetings will go live on November 17th. Additional details will be shared with you and your practices regarding the bundle as we get closer to the launch date.

Specialist Recruitment
  • Recruitment is open for specialist practices from 9/15/2023-11/3/2023. The Introduction to MCT2D for Nephrologists” recording is live at this link. Please send it to interested nephrology practices.
  • We are hosting an endocrinology webinar on 10/10 at 5:30-6:30. Please encourage your potential endocrinology sites to register for the webinar. Here is the link to register: Endocrinology Webinar: Tuesday, October 10th, 5:30pm-6:30pm
PO Scorecard
  • Ensure that all your clinical champions are registered for the regional meetings two weeks in advance of their individual meetings. We will send you a reminder three weeks in advance of the meeting if you still have unregistered sites, but these are viewable in the admin portal at any time.
  • PO monthly calls are taking place on Monday, October 9 at 11am or Wednesday, October 11th at 2pm. Ryan Carpus, MCT2D Senior Web Developer, will be giving a walk through of the administrative portal. As usual, please plan to attend one of the two meetings. Attendance at the monthly call is part of your PO Scorecard.
PCP VBR Requirements
  • Plan to convene your first meeting of clinical champions based on the VBR requirements- we are asking that you meet with your clinical champions to discuss the QI focus and make plans for practice level changes. This meeting needs to occur by 11/15/2023 and a target must be entered into the admin portal by that date as well.
Miscellaneous
  • The next PO Quarterly Reports will be distributed on 10/27. These new reports take into account the feedback and requests from the survey you completed.

Information to share with your participating sites:

  • Remind clinical champions to register for the regional meetings. You will be able to view who has already registered in the administrative portal. Ensure that all of your clinical champions or substitute attendees are registered within two weeks of their meeting.
  • Here is the link to the recording of the Patient Motivation learning community event that Dr. Gabison hosted in September. Physicians are able to meet their learning community requirement by attending a live event or watching a recording and completing the survey linked here.
