During our May PO calls (Wednesday May 8th and Monday May 13th) we will be asking for PO feedback on the following topics: the pre-populated reports that we will include as part of our dashboard redesign and our proposed performance measure that will begin on September 1, 2024. Please consider what type of standard reports would be helpful for you to view on the dashboard.

Collaborative wide meeting registration is closing soon, please plan to register by Friday, May 3rd so that we have an accurate count for the venue.
Learning Community Event: Friday, May 25th, 12pm-1pm- Member Panel on “Implementing CGM Interventions” where participants from physician organizations who chose to focus on continuous glucose monitors for their quality initiative focus will be sharing their experiences.
Please remind your providers about upcoming VBR deadline of June 1st to complete all primary care VBR requirements.
Next week we will be distributing an additional opportunity to meet the practice-level learning community VBR by submitting a brief survey about support needed related to DSMES, pre-diabetes, uncontrolled diabetes, and other topics.
The final component of the PO level quality improvement initiative VBR –“Lessons Learned/Reflection” is available for you to view in the admin portal. This is also due on June 1st but we wanted to give you some time to review at any meetings that you may have with practices prior to that due date.
POs with participating specialists: Details on the specific goal and approach for your PCP-SCP partnership are due in the administrative portal on June 14th. Prior to submitting these goals, the expectation is that you will have met with your specialist practice and your PCP practice, ideally together, to develop priorities and an approach to this partnership.
PCP recruitment for MCT2D closed April 19th. 59 new sites were enrolled in the program for a September 1st start date.
The Spring PGIP list has been imported into the MCT2D admin portal and changes in practice and physician alignment should be reflected there.
Version 1.0 of the virtual MCT2D Coverage Checker will be available at the end of May.
