A panel of MCT2D members across the state shared insights on practice- and PO-level quality improvement progress in implementing low carbohydrate lifestyle nutritional support and education for patients with type 2 diabetes. The discussion was moderated by MCT2D dietitian, Rina Hisamatsu, RDN, MPH. Rina shared initial findings from MCT2D's first patient-reported outcomes survey (PROS) on barriers to adopting and adhering to a low-carb lifestyle and opportunities to provide additional support to patients.

Our panelists responded to a variety of questions, including:
Explain the changes that your practices made around educating and supporting patients on a low carbohydrate diet.
Based on survey results, patients generally want ongoing support and follow-up from their care team. How did your PO incorporate this feedback into patient care and workflow?
What worked well for you in trying to make these changes? What was harder than you expected?
What changes related to low carb diet are you planning to implement next?
What is one thing you would like MCT2D to do to help you achieve your goals with regard to operationalizing this intervention for patients with T2D?
