Heidi L. Diez, PharmD, BCACP, MCT2D Co-Director, Lead Clinical Pharmacist. Dr. Diez is a clinical pharmacist specialist and clinical assistant professor in the University of Michigan College of Pharmacy. She graduated from the University of Michigan (UM) College of Pharmacy (2004) and completed a community practice residency at the University of Washington and Kelley Ross Pharmacy in Seattle, WA (2005). Prior to joining the University of Michigan in 2007, she practiced for two years in a partnership between the University of Washington instructor and Bartell Drugs, - as a clinical instructor and staff pharmacist. Over the past 13 years at UM, Dr. Diez has established four clinical practices in primary care at Michigan Medicine and coached other pharmacists across the state of Michigan in establishing primary care clinical services within her role in the Michigan Pharmacists Transforming Care & Quality (MPTCQ) Process Initiative sponsored by Blue Cross Blue Shield of Michigan. She is an active member of the University of Michigan Medical Group - Diabetes Quality Improvement committee and a leader in expanding CGM technology across primary care at UM. Dr. Diez also teaches courses in the management of type 2 diabetes at the University of Michigan College of Pharmacy, School of Nursing and Medical School.
