Patient Data Dashboard
Learn about using our population health dashboard
MCT2D Patient Data Dashboard is a population health dashboard focused on patients with type 2 diabetes in Michigan. The dashboard is designed to empower you with tools for quality improvement and population health management. With the dashboard, you can filter patients based on specific criteria, access summary data on MCT2D initiatives, and review detailed diabetes-specific patient health information.
DASHBOARD LOGIN
To access the patient data dashboard, you must be a member of MCT2D and have created a login. To request a login or reset your password, email mdc-accounts@med.umich.edu to request access to the MDC dashboard. It takes 3-5 business days to create a new user account and grant access.
