I think the set up of the form is easy for patients to understand - I think the form allows for patients to self reflect on their success of adhering to a low carb diet and exercise throughout the week as well
The tracker sheet is very detailed and allows for a lot of information to be documented. I liked that there is a carb guide attached to it. The carb guide is colorful and draws attention. It's really easy and simple to follow also.
