MCT2D is celebrating its third birthday! This year, we embarked on our first performance measure for primary care practices, achieving excellent glycemic control for our patients. This milestone marks a significant advancement in moving from the process of implementing our initiatives to assessing the outcomes that are most important to our patients. We also added two new physician organizations, 65 new primary care practices, and two endocrinology practices to our collaborative.
We are broadening the impact of our tools by transforming our coverage guide into an interactive web-based “Coverage Checker” and developing a new care team training program to expand access to low carbohydrate eating pattern education. We continue to advocate for removing barriers to care and have successfully collaborated with payers to expand continuous glucose monitor (CGM) coverage. We're sharing our successes inside and outside of Michigan, most recently at the Institute for Healthcare Improvement Forum, showcasing our advancements to a broader audience.
Looking ahead to 2025, we are thrilled about our upcoming work, particularly expanding our initiatives to include prediabetes. We plan to develop tools to clarify when and which patients to screen, how to get them into Diabetes Prevention Programs, and provide numerous patient education resources and guides. These efforts will continue to broaden our impact to achieve our vision — a world in which type 2 diabetes is no longer a progressive disease.
