
We are excited to announce that MCT2D will conduct individual practice visits with this spring’s regional meetings. We have heard that you want more one-on-one support from MCT2D. Through these site visits, we hope to gain a deeper understanding of your practice’s successes and challenges and how MCT2D can better support your team.
We plan to pilot these site visits with a small group of practices, one in each of the Kalamazoo (Grey Wolf), Grand Rapids (Bluegill), Saginaw (Riverwalk), and Traverse City (Sleeping Bear Dunes) regions.
How Site Visits Work
MCT2D program directors, program managers, and other team members will be on-site at your practice.
We aim to offer a flexible 4–5-hour window of time to meet as many staff members as possible, with the intention that the site visit will last between 2 and 4 hours, based on an agenda we create with your practice.
All staff are welcome to engage with MCT2D during the on-
site visit.
Individual staff members' time commitment for these visits will range. From a three-minute chat to an in-depth discussion or clinical walk through, your site can determine each member's level of involvement.
Possible visit activities/topics: Meeting practice leadership, discussing the current performance measure on A1c, addressing specific site challenges, and recognizing practice successes.
Participating in a site visit will meet your practice-level learning community requirement.
