Since welcoming our first cohort of Michigan physician organizations in July 2021, the Michigan Collaborative for Type 2 Diabetes (MCT2D) has catalyzed a paradigm shift in type 2 diabetes care across the state of Michigan.
We have a driving vision: To build a world where Type 2 Diabetes is no longer a progressive disease.
We are here to accelerate a new way forward through collaboration, innovation, and investment in quality, evidence-based care for type 2 diabetes. MCT2D strives to remove barriers to guideline-concordant care, support participating practices, and keep patients central to quality improvement work.
